When requesting a record from PTFD, we need to first know the nature and type of record you are requesting, this can be done through our e-mail system (yet to be determined) or by calling us at 574-255-5075. Once we determine what record you are needing we can then move forward with your request, please be patient as some records can be difficult to obtain to the legal procedures that must be followed by federal, state, and local laws.
